About LTFT programme
The LTFT training programme is designed to support GP trainees who are unable to train full-time due to personal circumstances. This option allows doctors to pursue their careers while balancing other commitments. LTFT trainees meet the same curriculum requirements as full-time trainees, though they work fewer hours, extending the training period proportionally.
Why work LTFT
- Work-life balance: Gives trainees time for responsibilities like childcare, caring for family, or health management.
- Continued professional growth: Enables trainees to maintain their training trajectory while managing other priorities.
- Flexibility: Allows completion of training at a pace suited to individual needs, though it may extend the overall training period.
Who can apply
- Trainees with a disability or ill health
- Trainees with caring responsibilities
- Welfare and wellbeing** – There may be reasons where trainees may benefit from a reduced working pattern. This could have a beneficial effect on their health and wellbeing
- Unique opportunities** – e.g., training for sporting events, or a membership of a national committee or continuing medical research
- Religious commitment**
- Non-medical development** – e.g., management courses, law courses or fine arts courses
- Flexibility for training and career development** with the option to train less than full time with flexibility that might enable development of a broad career portfolio

Things to Consider before application:
- Extended Training – Training duration will be longer due to reduced hours. Placements will be planned on a pro-rata basis (e.g., 0.8 WTE, 0.6 WTE)
- Placement Variability – There will be fewer opportunities to choose placements due to scheduling constraints.
- Reduced Pay – Pay will be reduced proportional to the number of hours worked. Check your estimated Salary as LTFT
How to apply for LTFT
- Inform your TPD: LTFT applications must be discussed with a Training Programme Director and submitted with adequate notice. Typically, a minimum of 16 weeks’ notice is required.
- Submit the form*: Complete and submit your LTFT application form which is available via the PGMDE Support Portal here
- Documents: Make sure to attach required supporting documentation as applications are reviewed for eligibility
*You must submit your application form as early as possible, no later than 16 weeks before starting your placement. If you are unable to provide 16 weeks' notice this will be reviewed on an individual basis. You must also provide written confirmation for your employing Trust that they can accommodate this.
**Applications must be submitted during the HET LTFT application window. Windows are determined by rotation months and can be found in the application form or Portal FAQs
What happens after I apply
- Your eligibility will be assessed, and you will be notified of the outcome
- It will be assumed you wish to continue training LTFT at the same WTE (whole time equivalent) until you inform otherwise.
- Submit a new application if you wish to change your working hours or return to full-time work. This application must be submitted within HET LTFT application windows/ at least 16 weeks prior to your rotation.
- If you are not eligible and wish to appeal the decision, you should arrange to meet with your programme director.
- If your application is unsuccessful you can appeal by submitting a ticket to the portal. This will be reviewed by your Head of School and Deputy Dean.
